The Geriatric Care Process
Our primary objective for Geriatric Care Managers is to establish a strong understanding of our clients. In our initial meeting, we conduct a comprehensive assessment to build trust and gather essential information. The Geriatric Care Managers focus on several key aspects, including reviewing the client’s medical history, documenting all medications with details on purpose, dosage, and frequency, evaluating physicians and other medical professionals along with their specific roles, assessing both physical and cognitive status, discussing advance directives and health care proxies, and evaluating home safety. Through this thorough assessment, we strive to create a holistic and personalized care plan tailored to the unique needs of each client.
CARE MANAGEMENT
Creation of a Care Plan and Personal Health Care Record
Within a few days of the initial assessment, you and your family will receive a formal document that summarizes our findings. It will contain a list of recommendations and our action plan in order to optimize care.
You will be provided with a USB flash drive that holds all pertinent information provided to us. If you ever encounter a medical emergency, responding doctors and other personnel will get a quick picture of your medical history. This can include documents such as who to contact during an emergency, durable power of attorney, and living will.
PERSONALIZED PLANS
Ongoing Care Management
After conducting an assessment and formulizing an individualized Care Plan, our Geriatric Care Manager will begin to put the appropriate services in place to follow our initial ideas. A sampling of services offered:
Medication Management: Ensuring the patient is taking his/her medications in the correct dosage and at the prescribed time. In addition, an RN can review of all medications to look for potential drug interaction.
Physician Relationships: Recommendation of the best local physicians for each client’s specific situation. Later, if desired, the Geriatric Care Manager will accompany the client to his/her appointments.
Acting as a liaison: The Geriatric Care Manager will serve as the bridge between multiple medical providers and families at a distance. The Geriatric Care Manager will always check in with every client’s family to make sure things are going well or to alert them to problems.
Counseling: Counseling is available to both the client and family to help them understand and adjust to the process of aging. If an alternative living option- such as a nursing home or assisted living facility- is necessary, the Geriatric Care Manager can help in placement assistance.
Crisis Management: Should an emergency occur, The Geriatric Care Manager will help provide distanced families peace of mind by seeing the client at a hospital or medical facility.
